• American Specialty Health

Note: It is the responsibility of the client to determine therapy coverage, co-insurance, and deductible amounts prior to scheduling an appointment. 

Our Billing Coordinator can provide you with CPT Codes needed to determine contracted rates directly with your insurance provider.

If you wish for us to NOT submit a claim to your insurance carrier, you have the option to be a Self-Pay Client. 

Rates for Self-Pay can be provided by calling the office 

at 630-320-6904. 

You may also request a Super Bill from our Billing Coordinator which you can then independently submit to your insurance carrier (if you are not in-network with any of the insurances listed to the left ) for reimbursement. Please note, this is subject to out of network costs and you will need to determine reimbursement rates directly with your provider.
